From the Nurse's Office...
To Parents of All Incoming 6th Graders
- All 6th grade students must submit a State of Illinois Health Examination form.
- This includes a current physical exam (dated within one year of the start of school), proof of immunizations, and the Health History section which MUST be completed by the parent/guardian.
- Forms can be downloaded at: http://www.wilmette39.org/highcrest/healthoffice.
- District #39 DOES exclude students who do not submit the needed form by October 15, 2011, so please get your child's form in to the Highcrest Health Office as soon as you can.
